Caring for Children: A Team Based Sport

Caring for Children: A Team Based Sport

Much has been written about team-based care in the last few years. Proactive outreach to patients and coordinating their care can be complex. Gone are the days of a patient presenting to your office for treatment and that is the totality of the interaction. We now have evidence that mental health and physical health are intertwined and social determinants of health have significant impacts on well-being. There is no way for a physician to meet all of the patient’s needs alone.

I was privileged to be part of a team at the American Academy of Pediatrics writing a policy statement on Team Based Care. We specifically wanted to make the point that the team needed to care for a child is distinctly different than the team needed to care for an adult with a chronic disease in the following ways:

  1. The most appropriate leader of the team for children is a pediatrician
  2. The required players on a child’s team change over time as the child grows and develops and their needs change
  3. All efforts at team-based care should keep the child and their family at the center

Please read more and feel free to reach out to me with your thoughts and comments you want them to look to you first. What is your strategy to create value for your medical home and make sure your appointments are filled?
